Registration & Pledge Forms
- The registration and pledge forms are attached, and can be downloaded. They are also available at various locations throughout the City.
- Registration forms must be received by February 25th, 2008, and can be dropped off at New Balance 4450 Walker Rd., Ambassador Bicycles 1980 Ambassador Dr., mailed to Spinergize 12382 Dube, Tecumseh, Ontario N8N 4N9 or emailed to info@windsorspinathon.com.
- For those participating in a Corporate or Organized group, the $400 sponsorship fee is due not later than February 22, 2008, and can be dropped of at New Balance 4450 Walker Rd., Ambassador Bicycles 1980 Ambassador Dr., or mailed to Spinergize 12382 Dube, Tecumseh, Ontario N8N 4N9. This is necessary to insure that the team shirts will be available for the event.
- All pledge money and pledge sheets must be submitted on the day of the event. Representatives from Hospice will be in attendance to assist with the collection. Pledges can be in the form of cash or check. Visa is also accepted on the day of the event.
- Pledges can also be submitted online at www.the hospice.ca/donations/index.php.
